Level Transtions?

This may be a stupid question, but how do I transition my maps? I've searched for this over and over and can't find anything, please help!

Thanks, simple tutorial that's very "to the point", and on my first test it worked. It should replace the Valve Dev wiki article, it confused me so much, I had no idea what it was talking about!

The valve dev wiki is designed to just tell you what something is, not to be a tutorial.
