Download :
extract and copy the files to
C:\Program Files\Steam\SteamApps*Username*\sourcesdk\bin\prefabs
Includes:
-Ball catcher
-Ball launcher
-button and door (includes switching indicator textures)
-portal cleanser + effect
-small observation room
-medium observation room
-large observation room

could you be more speciffic? I tested both just now and they work fine for me.
(if you have more than one pair you will need to rename every entity in the second pair)

thats just as unhelpfull.
I'm assuming by game you mean portal not hammer.
try putting just a catcher in, running it, then just a launcher and running that
to find out which one crashes it.
I've spent the last few days laboriously figuring things out and replicating the official stuff (i haven't used any decompiled maps, I don't know how to, I tried to use one ages ago but it never worked) I spent so long doing this I didn't do some school work and got in trouble.
Thanks alot. I'm sure alot of people will benefit from this. The lazy mappers of course.

Added a catcher and a launcher together and compiled... didn't crash
So i don't know what i did before... make they arn't meant to sit in the wall yes?

the launcher can go flush with the wall as it has a flat side, the catcher has to have a hole dug into the wall for the back part, if you sunk this into the wall without making a hole for it it may have crashed, don't know why though... leak perhaps?

use the entity placer, and on the right hand side of the screen where you select prop_static/info_player_start..etc just above that there is a drop down box that says entities, change that to prefabs
