vent.esports.ro:3999 (http://www.esports.ro/ventrilo/ <-- requires old version but u can request for admin)
I've got some other ones, but eh, I don't think the admins will like it.
But obviously someone could setup a free vent server if someone works at a datacenter for instance...
Even how to do something in the map editor or tell someone to test their map... it's like being on msn only you are using your voice instead of your fingers.

Fair point.
I vote for a ventrilo.

IRC seems to scratch this itch just fine, IMO.
TBH - People seem to forget that IRC is there.

Tell that to the 15-20 people who idle.

That's my point.
You go in mIRC with a problem, see loads of people "Woohoo people are in there, I can get my problem solved!"
You ask your question, and noone replies. People forget MIRC is on their taskbar. Even I do because there is no blink thing like MSN, not that it needs one.
At least with Vent you can hear people if theyre in and the chances are theyre not AFK.
That said, however, I'm of the philosophy that vent and IRC are not mutually exclusive.
Note: they don't just make you channels, they actually give you an entire ventrillo server for your use with full admin and possibly access on a FTP server.

And I'm more likely to be in IRC than a vent server just because it doesn't require a mic. I fire up IRC on my laptop while gaming and check it when I die, etc. Trying to carry on a conversation while defending the last point on Dustbowl isn't so easy.
I think vent server would be great, even if there wouldn't be alot of people. It would be cool to hear others talk. Another choice is our Steam Groups voice-chat. We could begin using it way more. (I don't know how much you have used it tho)
And chat would be more fun in Ventrilo than in IRC. I think it's totally different.
